    Our client has been providing both Residential Care and In Home Care to the elderly in NSW and ACT for nearly 50 years. With a focus on creating communities where seniors achieve their optimum quality of life, this organisation are leaders in their field and are now looking to appoint a Home Care Coordinator  who will play a pivotal role in maintaining their home care standards in Sydney Eastern Suburbs
      
    The Role:

    As a Home Care Coordinator, your main duties/responsibilities will include:
    • Coordinating a range of services to clients in their home environment
    • Managing a team of carers including assisting with performance appraisals 
    • Training and development of staff to ensure the highest care standards are maintained
    • Ensuring all client records and files are completed and actioned in a timely manner, maintaining privacy and confidentiality within legislative requirements and the organisation’s policies and procedures
    • Ensuring staff deliver excellent customer experiences for each visit, resulting in continuously improving positive customer feedback
    • Cultivating a “client directed approach” mentality within the team of carers
    • Completing client assessments and organising Home Care Packages (Level 1-4)
    The Ideal Candidate:
    • Ideally tertiary qualified in Social Science/Community Services/Frontline Management
    • Demonstrated experience in team management or supervision (Health & Community Services would be highly desirable)
    • Intermediate computer skills and data collection
    • Proven verbal and written communication skills including experience in writing reports and correspondence
    • Demonstrated ability to work collaboratively as a member of a team and to be self-motivated
